If you are submitting a new silhouette, please upload a clear side-profile photo of your child and provide the details exactly as you’d like them to appear. If you are using an existing silhouette on file, you may skip the upload or indicate which silhouette to use.

If you are ordering multiple silhouettes, please fill out a separate form for each child.

All birth stat information will be converted into longform text (e.g., “seven pounds, eight ounces” instead of “7 lbs 8 oz”) to match the heirloom style, but you are welcome to enter the basic information here.
